NEM-basierende Stegkonstruktionen können dank der CAD/ CAM-Technologie ohne aufkommende Spannungen hergestellt werden. Interessant ist in diesem Zusammenhang auch die Entwicklung der kunststoffbasierenden Komponenten. Dank der CAD/CAM Technologie ist es möglich, industriell vorgefertigte Kunststoffblöcke mit sehr guten physikalischen Materialeigenschaften weiter zu verarbeiten. Man spricht bei diesen Materialien auch von PMMA-basierenden Hochleistungspolymeren. Diese neue Generation der Kunststoffe erhält momentan eine enorm große Nachfrage zur temporären Versorgung von Defekten.
Der Einsatz temporärer Versorgungen ist ein wichtiger Arbeitsschritt im Rahmen indirekter restaurativer sowie interdisziplinärer Behandlungskonzepte. Sie dienen traditionell dem Schutz der beschliffenen Zahnhartsubstanzen und der Pulpa sowie der Stabilisation der Zahnposition. Zudem sind sie für den Erhalt oder die Rekonstruktion der Kaufunktion, der Phonetik und des ästhetischen Erscheinungsbildes unverzichtbar . Grundsätzlich werden nach dem Herstellungsmodus direkte, indirekte-direkte (Eierschalenprovisorium) und indirekte temporäre Restaurationen unterschieden.
Auf der Grundlage der durch den Herstellungsprozess bedingten Unterschiede in den Materialqualitäten werden verschiedene Tragedauern für die jeweiligen temporären Restaurationen empfohlen. Sie reichen von ein bis drei Monaten für die direkte Technik, bis zu 6 Monaten für die Eierschalenprovisorien und bis zu zwei Jahren für die indirekte Technik.

Keine Übernahme der Kosten für implantatgestützten

Auch die vollständige Zahnlosigkeit ist kein Garant für die Übernahme der Kosten für implantatgestützten Zahnersatz durch den Sozialhilfeträger. Vielmehr besteht weder ein Anspruch auf die Gewährung eines Zuschusses noch eines Darlehens zum Zwecke der Finanzierung des Zahnimplantates.

Zuvor hatte der ältere Mann bereits erfolglos eine Finanzierung der in einem Heil- und Kostenplan bezifferten Kosten von mehreren Tausend Euro durch seine Krankenversicherung angestrebt. Doch auch der zuständige Sozialhilfeträger lehnte eine Übernahme der Kosten ab, da hierfür vorrangig die Krankenkasse zuständig sei. Nachdem der daraufhin verklagte Sozialhilfeträger in erster Instanz Recht bekam, wandte sich der Kläger mittels Berufung an das Landessozialgericht (LSG) Baden-Württemberg.
Nach Ansicht des LSG Baden-Württemberg bestehe jedoch kein Anspruch auf eine Kostenübernahme für einen implantatgestützten Zahnersatz. Denn vom gesetzlichen Regelbedarf seien die Kosten für eine Versorgung mittels Zahnimplantaten nicht erfasst. Auch handele es sich nicht um einen im Einzelfall unabweisbaren gebotenen Bedarf. Vielmehr seien Kieferatrophien, die infolge von erheblichem Zahnverlust auftreten können, durchaus weit verbreitet und in der Praxis sehr häufig.

Berlin (dpa/tmn) - Die Zahnersatz-Wahltarife der gesetzlichen Krankenkassen sind keine ideale Lösung, um Geld zu sparen. Laut der Zeitschrift «Finanztest» können damit die Kosten beim Zahnarzt nur selten gedrückt werden. Grund: Die Leistungen sind sehr eingeschränkt.
Zwar gibt es meist keine langen Wartezeiten, bevor die Wahltarife in Anspruch genommen werden können. Doch weit kommt der Patient mit dem Geld laut «Finanztest» nicht: Es reiche allenfalls für den Zahnersatz, der der Regelversorgung entspricht. Sonderwünsche wie etwa Kronen aus Keramik statt aus einer Metalllegierung ohne Goldanteil zahlt der Patient weiter privat.
Für teuren Zahnersatz, wie Implantate sei eine leistungsstarke private Zusatzpolice das Beste, raten die Experten. Geht es nur um die Kosten der Regelversorgung, gebe es andere Lösungen. Zum Beispiel bieten etliche Kassen ihren Versicherten die Regelversorgung ohne Eigenanteil an, wenn sie bereit sind, bestimmte Zahnärzte aufzusuchen.

Wie strapazierfähig sind Veneers?

Ob man ein qualitativ hochwertiges Veneer erhalten hat, kann man allerdings frühestens nach fünf Jahren wissen. Perfekt hergestellte und angebrachte Veneers können 20 Jahre und länger halten, ohne sich zu verfärben.

Sind alle Voraussetzungen erfüllt und die Vorbehandlungen abgeschlossen, werden Abdrücke von beiden Kiefern genommen. Zunächst wird der Zahn, auf den später die Krone gesetzt werden soll, abgeschliffen. Es wird also rundherum Zahnsubstanz abgetragen. In weiterer Folge wird ein genauer Abdruck des abgeschliffenen Zahns genommen. Ein kleiner Abdrucklöffel wird dabei mit einer besonders weichen, anpassungsfähigen Masse gefüllt, die exakt die Form der Zähne und des beschliffenen Pfeilerzahnes wiedergibt. Anhand dieses Abdrucks erstellt der Zahntechniker im Labor ein Gipsmodell, auf dem er die Krone herstellt.
In der Zahnarztpraxis wird für die Zwischenzeit ein Provisorium angefertigt und mit Zement auf den Zahnstumpf gesetzt. Ein gut abschließendes Provisorium schützt den Zahnnerv vor Reizungen und mechanischen Belastungen. Bis zur nächsten Behandlungssitzung stellt der Zahntechniker die Krone her. Der Zahnarzt probiert sie ein und passt sie an. Die Krone muss harmonisch in den Zahnstumpf übergehen - ohne Ritzen oder Stufen. Ansonsten können sich am Kronenrand Zahnbelag bzw. Speisereste sammeln oder Keime eindringen. Der Zahnarzt prüft außerdem, ob sich der überkronte Zahn gut in die Zahnreihe einfügt und der Biss mit dem Gegenkiefer stimmt. Nach letzten kleinen Ver?nderungen wird die Krone endgültig zementiert oder eingeklebt. Die Lebensdauer einerKrone hängt vor allem von der Qualität der Zahnpflege ab.Vollgusskronen halten etwa zehn Jahre, während kunststoffverblendete Kronen schon nach fünf bis sieben Jahren unansehnlich werden können.  

Welche Arten von Kronen gibt es?

Vollgusskronen bestehen aus Gold oder anderen Metalllegierungen und sind sehr stabil. Aufgrund ihrer metallischen Färbung eignen sie sich nur für Zähne, die von außen unsichtbar sind (z.B. Backenzähne). Bei dieser Kronenart muss man am wenigsten Zahnsubstanz abtragen, um die Krone zu befestigen. Die erforderliche Metallschicht kann sehr dünn gearbeitet werden. Verblendkronen bestehen aus einem Metallkern, der mit zahnfarbenem Kunststoff oder Keramik überzogen ist. Der Zahn muss in diesem Fall stärker abgeschliffen werden als bei einer Vollgusskronen, weil die verschiedenen Materialschichten in Summe dicker sind. Kunststoff ist weniger stabil und langlebig. Das Material eignet sich deshalb nicht für die Kauflächen von Backenzähnen. Zudem verfärben sich Kunststoffverblendungen leichter als solche aus Keramik. Prinzipiell besteht bei verblendeten Kronen immer das Risiko, dass Teile der fragilen Verblendschicht beschädigt werden. Diese Gefahr gibt es bei metallischen Vollgusskronen nicht. Vollkeramik-Kronen sind die ästhetisch anspruchsvollste Lösung. Sie haben kein Metallgerüst, sondern bestehen komplett aus zahnfarbenem Porzellan. Geeignet sind sie vor allem für die gut sichtbaren Frontzähne. In allen ästhetisch relevanten Bereichen des Gebisses sind sie das Mittel der Wahl. Durch die Metallfreiheit wirkt das Ergebnis lebendiger, und bei einem späteren Zahnfleischrückgang werden auch keine unschönen Metallränder sichtbar.

Wann kommen Kronen zum Einsatz?

Bei einem stark vorgeschädigten Zahn lässt sich manchmal weder eine Füllung noch ein Inlay verankern. In diesem Fall eignen sich Kronen, die den Zahn wie ein Hütchen umfassen und seine Form, Funktion und die ursprüngliche Optik wiederherstellen. Eine Krone umgibt den abgeschliffenen Zahnstumpf rundherum, sie schützt und stabilisiert ihn auf diese Weise von außen. Teilkronen werden eingesetzt, wenn nur eine Hälfte des Zahns zerstört ist. Die äußere, sichtbare Seite des Zahns bleibt erhalten und damit so viel Zahnsubstanz wie möglich.
